Q Am I covered as a professional or amateur athlete and should I purchase the hazardous rider?
A No coverage for any organized professional or amateur sporting activities.

Q

One of the excluded items is emergency room charges for illness. Does that mean that I am not covered in the ER and are ER charges for injuries and accidents covered?
A The coverage does not cover going to the ER facility unless an overnight stay is required, then it is covered. Charges for services received at the ER (i.e. doctor/medicine) are covered for Illnesses and Injuries not otherwise excluded by the plan.

I will be traveling to a country that requires that I show proof of medical coverage and was wondering if my ID card is acceptable by the country's consulate?
A You may be required to produce a Visa letter, which is a document to substantiate that your coverage will be effective during your travels to a specific country. You may obtain a Visa Letter instantly by logging into HCCMIS's Client Zone at https://zone.hccmis.com/clientzone

I am a US citizen and will be applying to be a Canadian citizen in 3 to 6 months. However, I plan to come back to the states for a week. What do you recommend?
A When you purchase the Atlas Series for at least 3 months you are eligible for the Incidental Home Country Coverage, which will provide coverage in your home country during one incidental trip of up to 15 days.

Are pre-existing conditions covered? And can I be turned down for coverage?
A No, you will not be turned down, as the Atlas Series plans are guaranteed issued plans. There is a look back period for pre-existing conditions of 2 years prior to the certificate effective date for any pre-existing condition. The only exception is coverage provided under the Acute Onset of a Preexisting Condition benefit. Please refer to the Atlas exclusions page of this website for complete definitions.

If I purchase the Atlas Series for 12 months and end up coming back to my home country after 10 months, am I entitled to a refund of the difference in coverage I do not use?
A Yes.  Cancellation requests received after the effective date will be subject to the following conditions: (1) a $25 cancellation fee will apply; (2) only the unused portion of the plan cost will be refunded; and (3) only members who have no claims are eligible for cancellation.

If you desire worldwide coverage, including coverage in the US and Canada, then Coverage Area Option 1, Including the US and Canada, is suitable for you. Can I pay monthly for my coverage?
A  Yes. Payment options are annual, semi-annual, quarterly, and monthly (when choosing monthly payments, the first payment will be for two months).

I am a US citizen living in Spain, however I will be returning to the US for period of 25 days in December for the holidays. What are my options?
A With the Citizen Secure plan, you can travel back to the US, as long as you are living abroad for at least 6 months.

Is there a provider listing of hospitals or doctors affiliated with this health plan in the US? Is it required to use these preferred providers?
A Yes. Go to Hygeia.net for an online listing of providers. It is not necessary to use these providers, but you may have usual and customary charges associated with non-network providers in the US.

Is there a network outside the U.S.?
A There are a number of providers which offer direct payments and negotiated discounts. However, it is not necessary to use specific providers outside the US.

What is the policy on pre-certification of emergency hospitalization?
A You will need to contact HCC Medical Insurance Services to pre-certify as soon as possible before a scheduled surgical or planned hospitalization, with in 48 hours of an emergency hospitalization or within the first 90 days of pregnancy.

How can I pre-certify for medical services or hospitalization when I am involved in an accident or injury?
A In the case of being admitted to the hospital, due to an accident or injury, pre-certification is required within 48 hours of being admitted to a hospital.

Are Prescription Drugs covered?
A Yes, as eligible medical expenses covered under your plan, subject to deductible and coinsurance.

Q

If my application is not accepted, is the premium refundable?
A Yes, since the Citizen Secure plan is fully underwritten, and if an offer for coverage is not available, then no charges will be applied on your credit card.
Q How do I file a claim?
A Filing a claim is easy. Once your Application is accepted, you will receive a kit which contains claim forms. Just complete the claim form, attach original, itemized bills and forward them to HCC Medical Insurance Services, Inc. Be sure to complete your claim form, sign it, and indicate a convenient time and location to contact you in the event questions arise. If you have already paid certain expenses, attach a copy of your paid receipt. You will be reimbursed, in many cases, HCC Medical Insurance Services will make payment directly to the hospital or physician who treated you. Remember, you are responsible for the deductible, coinsurance and any uncovered charges.
